I as the parent/guardian agree that all mentioned below apply to me, my child, or my children.
I agree and consent to the following:
I am voluntarily participating in the classes conducted by Dance Fitness Academy & Healing Arts. I recognize that some of the classes like exercise or dance classes require physical exertion that may be strenuous at times and may cause physical injury and I am fully aware of the risk and hazards involved.
I understand that it is my responsibility to consult with a physician prior to my participation in any class from DFA&HA.
I agree to assume full responsibility for any risk. Injuries or damage known or unknown which I might incur as a result of participating in any of the classes/ programs/ courses, workshops, events, Participating in Performing and community events, etc. Such injuries may include. But are not limited to, heart attack, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, beat prostration, injuries to knees, injuries to the back, injuries to the foot, or any other illness or soreness, including death.
I promise not to hold the Organization, DFA&HA, or My Dance and Fitness Studio Owner, the Company, its officers, agents, employees, or Instructors, liable or pursue legal action against them. This waiver absolves the Organization of any duty for injuries sustained on the premises before, during, or after the activity. By signing this agreement, I agree to hold the DFA&HA and My Dance and Fitness Studio completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances.
I waive this location mentioned above for Me and My heirs or representatives forever release waive discharge and convent not to sue the Organization DFA&HA or the owner Of My Dance And Fitness Studio.
I have read the above waiver and release of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
Photo and Video: I understand pictures and videos may be taken at any time and used for Publicity and advertisement purposes.
Covid 19.
By signing this agreement, I acknowledge the contagious nature of COVID-19, and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending this location and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
